Commando Factory – Member Waiver & Release Form

Assumption of Risk

I understand that participation in physical exercise, training, and fitness activities at Commando Factory involves inherent risks, including but not limited to muscle strains, sprains, falls, accidents, illness, equipment failure, and other unforeseen injuries. I voluntarily choose to participate and accept full responsibility for any risks to my health, safety, and well-being.

Medical Clearance

I confirm that:

• I am in good physical condition and have no medical condition or injury that would prevent me from safely participating in gym activities.

• I have consulted a doctor if necessary and will inform the coaches of any changes to my health.

• In the event of illness, injury, or accident, I authorise Commando Factory staff to seek emergency medical assistance on my behalf if required.

Release of Liability

By signing this agreement, I:

• Waive, release, and discharge Commando Factory, its owners, staff, and affiliates from any and all liability for injury, accident, illness, loss, or damage arising from my participation, except where such liability arises directly from the proven negligence of Commando Factory or its staff.

• Understand that this waiver applies to risks associated with physical activity, equipment use, and facility conditions, to the fullest extent permitted by law.

Personal Belongings

I understand that Commando Factory is not responsible for any lost, stolen, or damaged personal items brought onto the premises.

Rules & Conduct

I agree to:

• Follow all safety instructions given by coaches and staff.

• Use equipment properly and with respect.

• Respect other members and the facility.

• Not allow anyone else to use my membership.

Payments & Membership

I understand that:

• Membership fees are non-refundable except where required by law.

• Late or failed payments may result in suspension of membership.

• Cancellation policies are outlined in my membership agreement.

Photography & Media Consent (Optional)

Photography & Media Use
From time to time, photos and videos may be taken during training sessions for use on Commando Factory’s website, social media, and marketing. If you would prefer not to appear in any photos or videos, please let us know and we will ensure your wishes are respected.

Acknowledgement

I confirm that I have read, understood, and voluntarily signed this waiver and release form. I understand it is a condition of membership at Commando Factory.
